The easiest and most secure way to complete your forms is through our online patient portal, powered by Tebra. Once your appointment is scheduled, you will receive an invitation to create your portal account where you can fill out all necessary documents electronically.

If you prefer to fill out your forms by hand, you can download them below. Please print, complete, and bring them to your appointment.

Alternatively, you may email the completed forms to us at [email protected] before your scheduled visit.

Preparing for Your First Visit

In addition to your completed forms, please be prepared to provide the following at your first appointment:

Your photo ID

Your current insurance card

A list of all current medications and dosages

Any relevant medical records you may have
